Documents for Entry

In case you would like to become a Member of MKB Health Fund, please fill out our Statement of Entry (Belépési nyilatkozat) and send it to our mailing address (MKB Egészségpénztár, 1821 Budapest)!

MKB Health Fund is adding a clause to the Statement of Entry after receipt of the document. Your membership is active from the date the clause was added, and from that date you may use the services of MKB Health Fund as well. A copy of the Statement of Entry is returned to you including your Membership ID. (In case of employer contribution, a copy is sent to the Employer as well.)

If your employer supports you with employer contribution, please have your Statement of Entry stamped by your HR department. (For employer contribution an Agreement on employer contribution is needed between your employer and MKB Health Fund.)

We suggest you to fill out the document Persons entitled (Szolgáltatásra jogosultak) to services and submit it with the Statement of Entry to the Fund to enable a more extensive use of services. (The list of Persons entitled to services may be completed or modified anytime during membership.)

In case you determine a Beneficiary in case of death, the beneficiary(ies) in case of death will receive the balance of the account according to the percentage determined previously. If Fund Member does not determine beneficiary in case of death than inheritance will follow the legal procedure of inheritance.

If you are already a member of a Health Fund and you would like to join MKB Health Fund, please fill out and submit both Statement of Entry and Transfer (Átlépési nyilatkozat) documents.

Documents for reimbursement

For submitting an invoice paid in cash please always use our Application form (Igénybejelentő nyomtatvány)!

For legal use of certain services, Government Decree specifies the use of medical recommendation (orvosi igazolás).
Medical recommendations have no formal requirements; however, it is expedient and recommended that members and physicians should use the Medical recommendation code sheet available at the website. Medical recommendation is valid for 5 years, so there is no need to send a medical recommendation for every single service use separately.

It is possible to apply for Fund (munkáltatói szerződés) services in case of Fund Member\'\'s disability to work - including the disability to work because of child\'\'s illness - debited to the individual health account. (According to the law, only Fund Members are entitled to reimbursement for loss of income, persons entitled to service are not.)
Accounting for loss of income due to illness commence from the first day of disability to work. Maximum amount of service is the amount of loss of income certified by the employer. (In case of passive sickness benefit both employer\'\'s and National Health Insurance Fund\'\'s certificates are needed for accounting.)

Documents for reporting data changes

If any of your membership data have changed (adatváltozások bejelentése) in any manner (permanent address, mailing address, bank account number is changed; persons entitled to service, beneficiary in case of death are admitted/deleted, etc.) please report it in writing directly to the Fund.

All changes in personal details may only be accepted by the Fund in the form of the original counterpart of a statement duly signed or on a designated form.

Documents for application for a Health Card

After the 1st of June 2007 MKB Health Fund provides all of its Members with a free cash saving Health Card automatically after entry.
If you joined the Fund before 2006, or did not apply for a Health Card before the 1st of June 2007 and haven\'\'t got any Health Card yet, please download the document Application for a Health Card from our website, and MKB provides You with a free Health Card.

For the registered persons entitled to services it is possible to apply for a partner card by using the Health Card Application Form.

The price for the partner card is 2000 HUF, which is automatically debited to Fund Member\'\'s individual health account on the evidence of the Application.
If Fund Member has lost his/her Health Card or Health Card has to be changed because of name alternation, Fund Member may apply for a new Health Card - after arresting the original Health Card. New Health Card costs 2000 HUF, which is directly debited to Fund Member\'\'s individual health account on the evidence of the Application document.

Other documents

After filling out and submitting the document Statement on Email notification Health Fund sends a report on Fund Member\'\'s individual health account and the latest information on Health Fund to the Fund Member\'\'s email address on a monthly basis (currently only hungarian).
